Equipment Reservation Guidelines 1. Equipment must be reserved by 4:00 p.m. the business day prior to use. 2. Students must fill out the equipment reservation form on eiucomm.net. There is an ?Equipment Reservation" link on the left side navigation menu. Students must indicate a time they will pick up the camera. The time must be during open lab hours which are listed below. When picking up equipment, students will be asked to specify a return time. 3. Reservations are made on a first come, first served basis. 4. Students should use their University e-mail account. Lab staff is not responsible for lost acceptance and rejection e-mails. 5. Requests for equipment without a reservation will not be honored. 6. The person making the reservation must be the one checking out the equipment. 7. Students may make more than one reservation at a time. If however, the pattern of reservations becomes abusive or is unfair to other students, the lab manager may deny some or all of the reservations. 8. Incomplete reservations will not be honored. This includes reservations that do not have pick-up times; reservations made when the lab is not open for equipment pick-up; and when reservations are made after 4:00 p.m. the day before. 9. Equipment may be picked up during open hours. Open hours are listed on the website and outside of the lab. 10. A valid student ID must be presented when picking up and dropping off equipment. The name on the ID must be the same as the name on the reservation. If no ID is available, the reservation is void. 11. Students may have equipment in up to four (4) hour increments. 12. If a student reserves equipment and fails to pick it up or fails to cancel a reservation twice during the semester, equipment privileges will be suspended. 13. Students who return equipment late more than two times will have equipment privileges suspended. (See Student Problem Report) 14. Reservations will not be honored if a student does not appear during the 10 minute pick-up window. 15. Students are responsible for the equipment if it is lost, stolen, or damaged. Equipment is checked prior to check-out and then again when it is returned. 16. Equipment may only be used for an assignment in a communication class or a sponsored departmental activity. Any other use must be approved by the department chair.
